Community Support Solutions of Washington (CSSOW) In Home Long Term Care provides Support Services in the Care recipients home or in the community they live.
As a Medicaid, Personal Care Agency we work with Washington State with Division of Developmental Administration (DDA) caseworkers and the Care Assessment they provide us to ensure Long Term Care provides the services and supports care recipients and their families need.Long Term Care is for those who say they would prefer to live in their homes and participate in their communities and have a balanced and enriched life.
Community Support Solutions In Home Long Term Careworks to support community living and independence for people with disabilities in our Communities based on the belief that all people can live with dignity, make their own choices, and participate fully in society. We provide this service by providing quality trained, screened and caring caregivers.
In Home Long Term Care CSSOW provides is contracted through (DDA) Services include Respite, Support Services , and Companion Care. The population we work for is represented by our contracts with Division of Developmental Administration (DDA) who provides care coordination for any individual who has a developmental disability that starts before 18 years of age. DDA clients include children as well as adults who may have developmental delay, cerebral palsy, epilepsy, autism and other neurological conditions. DDA also oversees the Medicaid personal care funding of children with special needs under 18 that are not considered developmentally disabled for example medically fragile.
Philosophy
Community Support Solutions Long Term Care provides care support in collaboration with the client and their families. We believe that the client and primary caregiver is the best source of information on the care of the client and, as such, acts in a primary resource role to the support provider.
